INTAKE COORDINATOR

COMMUNITY REFERRAL COORDINATOR

This position will serve as the Community Referrals Coordinator for the telephonic Coordinated Intake and Referral (CI&R) unit and will be responsible for the receiving and tracking of all referrals and assuring service coordination and communication with referral sources.
