Healthy Start Risk Screening

The Healthy Start Prenatal Screening Form is a simple, brief questionnaire that helps identify factors in a pregnant woman’s life including medical and social issues that might lead to poor pregnancy outcomes such as low birth weight, premature birth, or developmental delays of the child. This screening is used throughout Florida and is funded by the Florida Department of Health.

Florida Statute 383.14 requires that all pregnant women be offered the Healthy Start Prenatal Screening at their first or consequent prenatal visit by their healthcare provider.  The Healthy Start Infant (Postnatal) Screening must be offered to parents or guardians of all infants born in Florida before leaving the delivery facility.

After the screening forms are sent to the local health department from your office, women who consent to be screened and are referred by your office will be contacted by a specialist from our Connect team. Connect is a telephonic service where eligible participants are contacted, assessed for risks, provided information and resources, and may be referred to home visiting programs and other community services based on their risks and need.
