Broward Healthy Start Coalition is providing notice that IRS Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, for the 2025 calendar year is available upon request.

Form 1095-C contains information about the health insurance coverage offered to you during the year and may be needed when preparing your federal income tax return.

Eligible employees and former employees may obtain a copy of their Form 1095-C by:
