This position will serve as the Community Referrals Coordinator (CRC) for the Telephonic Coordinated Intake & Referral (CI&R) unit, responsible for receiving and tracking all referrals, coordinating services, and maintaining communication with referral sources.
As an Intake Assistant, you’ll help guide families to essential support services during pregnancy and early childhood. You’ll process high‑volume referrals and provide key administrative support within our CI&R team. Your work directly strengthens healthier outcomes for families across our community.
This role serves as the Quality Assurance Specialist for the Telephonic Coordinated Intake and Referral (CI&R) unit, ensuring accuracy and compliance in documentation and procedures. The position supports the Connect referral process, linking pregnant women, interconceptional women, and families with young children to essential services that reduce risk factors for poor health and developmental outcomes.
